You would normally one need to click each of the bookmarks that do not have a favicon to make Firefox retrieve the favicon from the server.Whether Flash is up to date or outdated shouldn't have any effect.favicons are lost when there is a problem with the favicons.sqlite database or with the places.sqlite database (bookmarks and history).
