This installation was a little weird because the install download was not an executable file. I clicked on Oracle_VM_VirtualBox_Extension_Pack-5.1.4-110228.vbox-extpack and selected 'Select a program from a list of Installed programs' and than selected Oracel virtualbox and it installed the extension. That fixed the problem, but another less desirable solution is you can disable the usb.
