I recently was developing an MS Word file (a client report) which I started while in the connected world and, as a result, sync'd to OneDrive. I think continued to work on that file in off-line mode while on a long plane flight and for much of the following day as I was away from the office and enable to connect to the internet (and yes, I had exited and saved it multiple times). Upon completion of the file, I re-connected to the internet as I was preparing to email the final document to my client. To my surprise, when I went to attach the document to my email for transmission. The version available to me was the pre-flight copy of the document. Apparently, during the auto-connect process to Office 365, the OneDrive synchronisation process decided to download the older version "in the cloud" to my computer rather than upload the newer on my computer (in the off-line folders) to the OneDrive cloud storage. How any synchronisation algorithm could allow that to happen escapes me but it most assuredly happened.
